Youth Volunteer Waiver (General)

Volunteers under age 18, please have a parent or guardian complete and sign this Youth Volunteer Waiver for you, which is required before you can volunteer at Solid Ground. If you plan to volunteer at Marra Farm, please fill out the Youth Waiver for Marra Farm Volunteers instead. PLEASE NOTE: Youth under age 14 must be accompanied by an adult while volunteering, and all adult volunteers must submit a One-Time Volunteer Intake Form or Volunteer Application Form as well.

  • I certify that the answers given above are true and complete to the best of my knowledge. I authorize investigation of all statements contained in this waiver as may be necessary for ensuring a satisfactory volunteer experience. I understand that this waiver is not, and is not intended to be, a contract. I understand that false or misleading information provided in this waiver may result in our not being able to continue volunteering with Solid Ground.
  • I hereby give permission that the above youth(s) may be given basic first aid treatment as needed by staff members at Solid Ground. I recognize that participation in outdoor gardening activities, peer tutoring, and other volunteer projects, even when well supervised and managed, pose a risk to the youth(s), and I agree to assume such risk on their behalf. In the case of injury, I authorize Solid Ground staff to administer basic first aid and/or obtain whatever medical treatment they deem necessary for the welfare of my child. I further agree that I will be financially responsible for all charges and fees incurred in the rendering of said treatment, regardless of whether my medical insurance covers such charges and fees. I, the undersigned, hereby release, indemnify, and hold harmless Solid Ground, its officers, directors, and employees, and the organizers, sponsors, and supervisors of all Solid Ground activities from any and all liability in connection with any injury that my child may sustain (including any injury caused by negligence) in conjunction with volunteering with Solid Ground.
  • In checking YES or NO and signing below, I indicate whether or not the youth(s) listed above may be photographed, videotaped, and/or recorded while volunteering with Solid Ground. I understand that Solid Ground will own rights to and may use this media (photographs, videos, audio recordings, and/or my statements), in whole or part, in Solid Ground materials such as printed publications, the Solid Ground website (www.solid-ground.org), videos, social media, grant proposals, and promotional materials to support Solid Ground and its programs. As far as I know, what I say and do in this media will not violate the rights of any other person or company. If I no longer want these photos and/or story to be used, I agree to contact the Solid Ground Communications Department at publications@solid-ground.org or 206.694.6716. Once requested, Solid Ground will not create new materials using participants’ media – but we may continue to use already printed materials until we can make replacements.
